The Intermediate Coronary Branch supplies blood to the heart muscle, branching from the left main coronary artery.

Intermediate Coronary Branch, also known as the Ramus Intermedius, is a variant coronary artery arising from the left main coronary artery (LMCA). It occurs when the LMCA trifurcates instead of the typical bifurcation into the left anterior descending (LAD) and left circumflex (LCx) arteries. This intermediate branch lies between the LAD and LCx and supplies a distinct myocardial territory.


Origin

The Intermediate Coronary Branch originates directly from the left main coronary artery, usually at its distal end where the LMCA divides. Instead of a bifurcation into two branches, the LMCA trifurcates into three branches: the LAD, the LCx, and the Intermediate branch itself. This variant occurs in approximately 15–30% of the population.


Course

The Intermediate Coronary Branch courses along the anterior interventricular sulcus or the atrioventricular groove, depending on its size and dominance. It typically runs between the usual pathways of the LAD and LCx arteries, following a variable trajectory that can resemble either one of these arteries or a distinct path. The course is usually short to moderate in length, but this can vary with individual anatomy.


Branching Pattern

The Intermediate Coronary Branch gives rise to several smaller branches that supply specific myocardial regions. These branches may include diagonal branches similar to those arising from the LAD, or obtuse marginal-like branches akin to those from the LCx. The exact branching pattern is highly variable and depends on the size and course of the intermediate branch.


Termination

The Intermediate Coronary Branch generally terminates after supplying its myocardial territory, often an area of the lateral left ventricular wall or the anterolateral papillary muscle. Termination may occur by tapering off into smaller branches or anastomosing with branches of the LAD or LCx arteries.


Territorial Supply

The territory supplied by the Intermediate Coronary Branch usually includes portions of the anterolateral wall of the left ventricle, an area that might otherwise be perfused by diagonal branches of the LAD or obtuse marginal branches of the LCx. Its presence can influence coronary perfusion patterns and is important in coronary artery disease assessment and intervention planning.


Left Main Trifurcation Context

The presence of the Intermediate Coronary Branch modifies the typical coronary anatomy from a bifurcation to a trifurcation pattern at the distal LMCA.
