11.4 Anterior Interventricular Artery
The Anterior Interventricular Artery supplies blood to the front of the heart, playing a key role in cardiovascular function.
Anterior Interventricular Artery is a critical coronary artery branch primarily responsible for supplying oxygenated blood to the anterior portion of the interventricular septum and adjacent areas of the left and right ventricles of the heart. It is also known as the left anterior descending artery (LAD), reflecting its origin and course along the anterior interventricular sulcus. This artery is a major branch of the left coronary artery and plays a vital role in myocardial perfusion, especially of the anterior wall of the left ventricle and the interventricular septum.
Origin
The Anterior Interventricular Artery originates from the left coronary artery immediately after the latter emerges from the left aortic sinus. It arises as the continuation or one of the principal branches of the left coronary artery and descends obliquely toward the apex of the heart within the anterior interventricular sulcus. This origin is anatomically positioned between the pulmonary trunk and the left atrial appendage.
Course
Anterior Interventricular Sulcal Course
The artery travels in the anterior interventricular sulcus, a groove on the sternocostal surface of the heart between the right and left ventricles. It extends from the base of the heart toward the apex, lying superficially within this sulcus, which provides a defined path for the vessel. The sulcus serves as a landmark to locate the artery during surgical or diagnostic procedures.
Proximal Anterior Interventricular Segment
The proximal segment is the initial portion of the artery descending from its origin, characterized by giving off important branches such as septal perforators and diagonal branches. This segment is larger in diameter and supplies the upper anterior interventricular septum and adjacent ventricular walls.
Mid Anterior Interventricular Segment
This middle portion continues along the sulcus, maintaining a descending trajectory. It gives rise to additional septal and diagonal branches that extend laterally to supply the muscular interventricular septum and anterior free wall of the left ventricle.
Distal Anterior Interventricular Segment
The distal segment approaches the apex of the heart, tapering in diameter. Near the apex, it curves around the cardiac margin and forms anastomoses with the posterior interventricular artery, contributing to collateral circulation.
Branches
First Diagonal Branch
One of the principal branches of the anterior interventricular artery, the first diagonal branch, arises from the proximal or mid segments. It courses obliquely over the anterior surface of the left ventricle to supply the anterolateral wall. This branch is significant for perfusing the left ventricular free wall.
Additional Diagonal Branches
Beyond the first diagonal, several smaller diagonal branches may arise along the artery’s course. These branches distribute blood to the lateral aspects of the left ventricle and contribute to the extensive vascular network supplying the myocardium.
Septal Perforating Branches
Numerous septal branches arise perpendicularly from the anterior interventricular artery, penetrating the interventricular septum. These branches supply the anterior two-thirds of the interventricular septum, including the bundle branches of the conduction system.
Apical Arterial Course and Termination
The artery continues toward the apex, where it typically narrows and terminates by anastomosing with the posterior interventricular artery, which arises from the right coronary artery or sometimes the circumflex artery depending on coronary dominance. This anastomosis facilitates collateral circulation between the anterior and posterior interventricular regions.
Functional Significance
The anterior interventricular artery is critical for myocardial oxygenation, especially of the left ventricle, which performs the major workload of systemic circulation. Occlusion or stenosis of this artery, particularly the proximal segments, can result in extensive anterior wall myocardial infarction, often referred to as the "widowmaker" infarction due to its severity. The artery’s branches ensure perfusion of the interventricular septum, crucial for the cardiac conduction system integrity.
