Third Plague Pandemic was a global outbreak of bubonic plague that began in the Yunnan province of China in the mid-19th century and spread worldwide from the 1890s into the 20th century. It was caused by the bacterium Yersinia pestis and marked the third major historical wave of plague, following the Justinianic Plague and the Black Death. The pandemic resulted in over 12 million deaths, with India being the most severely affected. It was also the first pandemic to occur during an age of rapid global transportation and scientific bacteriology, leading to new understandings of disease transmission and new public health measures.


Origins and Early Spread

Yunnan and South China

The roots of the Third Plague Pandemic trace to the uplands of Yunnan in southwestern China, where endemic plague in rodent populations occasionally spilled over into humans. The combination of ecological disruption from mining, war, and migration in the late 19th century increased human exposure. By the 1850s–1870s, outbreaks surfaced in Yunnan towns, gradually moving eastward through trade and transport connections.

Arrival in Hong Kong (1894)

In May 1894, a major outbreak struck Hong Kong, then a British colony and a major port. The city’s dense, international population and shipping traffic facilitated global dissemination. Thousands died in a matter of months, and the event triggered worldwide alarm, spurring international cooperation and scientific investigation.

From Hong Kong and southern Chinese ports, plague reached Bombay (now Mumbai) in 1896. The disease spread rapidly across India, especially in crowded urban slums and among migrant laborers. Over the next two decades, millions of deaths occurred, with periodic resurgences into the 1920s. The British colonial government implemented aggressive anti-plague measures, including quarantines, house searches, and forced hospitalizations, often provoking resistance and unrest.

Indian Ocean, Africa, and Beyond

Shipping routes carried plague to the Indian Ocean ports, the Arabian Peninsula, Egypt, and East and South Africa. By the early 1900s, outbreaks occurred in Madagascar, Mauritius, and as far as Cape Town. Plague reached port cities on every continent except Antarctica, including Sydney, San Francisco, Honolulu, and Buenos Aires. Urban outbreaks sometimes seeded rural endemic foci.


Scientific Breakthroughs

Pathogen Discovery

The Hong Kong outbreak coincided with the rise of bacteriology. In June 1894, Alexandre Yersin, working for the Institut Pasteur, isolated the plague bacillus, later named Yersinia pestis. This discovery enabled scientists to trace transmission routes and develop diagnostic tools.

Plague Ecology: Rats, Fleas, and Reservoirs
