3.10 Inferior and Superior Cardiac Relations

Inferior and superior cardiac relations refer to the anatomical connections between the heart and surrounding structures in the thoracic cavity.

Inferior and Superior Cardiac Relations refer to the anatomical relationships of the heart and its pericardium with the structures that lie inferior (below) and superior (above) to them within the thoracic cavity. These relations are of critical importance in understanding cardiac anatomy, surgical approaches, and the spread of disease processes. Inferior and superior relations involve direct contact surfaces, vascular continuity, and fascial connections between the heart, pericardium, diaphragm, great vessels, and mediastinal structures.

The inferior (diaphragmatic) surface of the heart rests directly on the central tendon of the diaphragm. This surface is mostly formed by the left ventricle, with a smaller contribution from the right ventricle. The fibrous pericardium, which encloses the heart, is firmly attached to the central tendon of the diaphragm, anchoring the heart and limiting its movement during respiration.

Inferior Cardiac Surface-Diaphragm Relationship

The surface of the heart in direct contact with the diaphragm is known as the diaphragmatic or inferior surface. It is separated from the abdominal cavity by the diaphragm. This relationship is clinically significant in procedures involving the subxiphoid approach to the pericardium and in the spread of infections or fluids between the pericardial and peritoneal spaces.

The ascending aorta arises from the left ventricle and is enclosed within the pericardium for a short distance before entering the superior mediastinum. Its proximity to the pulmonary trunk and superior vena cava is key to the arrangement of the transverse pericardial sinus, an important landmark in cardiac surgery.

The pulmonary trunk emerges from the right ventricle and passes superiorly and posteriorly, lying to the left of the ascending aorta. It is enveloped by the pericardium and, along with the aorta, forms the anterior boundary of the transverse pericardial sinus.

The superior vena cava (SVC) is partly encased by the pericardium as it descends to enter the right atrium. The pericardium fuses with the adventitia of the SVC, and the vessel lies to the right of the ascending aorta, anterior to the right pulmonary artery.
