Humoral Medicine

Humoral Medicine was a historical system of medicine based on the balance of bodily fluids, shaping medical practices across cultures and centuries.

Humoral Medicine is a system of medical theory and practice that dominated Western and Islamic medicine from antiquity until the early modern period. It is based on the belief that the human body contains four fundamental bodily fluids or "humors": blood, phlegm, yellow bile, and black bile. Health is understood as a state of balance or equilibrium among these humors, while disease arises from their imbalance or dyscrasia. Diagnosis and treatment aim to restore this balance through various methods including diet, regimen, bloodletting, purging, and other interventions designed to adjust the humoral composition.


The Four Humors

Blood

Blood was considered the dominant humor, associated with the element air and characterized by qualities of warmth and moisture. It was linked to a sanguine temperament, which was thought to make individuals cheerful, active, and optimistic. Excess blood was believed to cause symptoms such as fever and inflammation.

Phlegm

Phlegm corresponded to the element water and was cold and moist in nature. It was associated with a phlegmatic temperament, marked by calmness and lethargy. Excess phlegm was thought to result in sluggishness, respiratory issues, and mucus-related illnesses.

Yellow Bile

Yellow bile aligned with the element fire and was warm and dry. It corresponded to a choleric temperament, characterized by aggression and ambition. Surplus yellow bile was linked to fevers, irritability, and digestive disturbances.

Black Bile

Black bile was connected to the element earth and had cold and dry qualities. It was related to a melancholic temperament, involving sadness and introspection. An excess of black bile was believed to cause melancholia (depression) and digestive problems such as constipation.


Temperament and Disease

The humoral theory proposed that each individual’s temperament was determined by the predominant humor in their body. Temperaments influenced physical constitution and psychological traits, as well as susceptibility to certain diseases. For example, a sanguine person was robust and energetic, while a melancholic person was prone to mood disorders.

Disease was seen as a manifestation of humoral imbalance. Physicians assessed symptoms in light of the patient’s temperament and the prevailing humor causing disturbance. The goal was to tailor treatments to the patient’s specific humoral disposition.


Humoral Imbalance and Diagnosis

Illness resulted from excess, deficiency, or corruption of one or more humors. Diagnosis involved careful observation of bodily signs such as the pulse, urine, complexion, and bodily secretions. Two key diagnostic techniques were:

Pulse Examination

Physicians examined the pulse’s strength, rhythm, and quality to infer the state of the humors, the vitality of the patient, and the stage of illness.

Urine Examination

The color, consistency, sediment, and other properties of urine were analyzed to detect humoral imbalances and guide treatment decisions.


Treatment Methods

Bloodletting

Bloodletting was used to reduce excess blood or other humors. It was one of the most common and established treatments, believed to remove harmful substances and restore balance.

Purging and Emetics

These methods aimed to expel surplus humors through vomiting or bowel movements, especially to eliminate excess bile or black bile.

Diet and Regimen

Dietary recommendations were central, focusing on foods and drinks classified by their humoral qualities (hot, cold, moist, dry). Lifestyle and environmental factors such as exercise, sleep, and climate were adjusted to support humoral equilibrium.


Climate and Humoral Health

Humoral medicine emphasized the influence of climate and seasons on health and temperament. Hot, dry climates were thought to increase yellow bile, while cold, wet climates favored phlegm accumulation. Physicians advised patients to adapt their habits and treatments to environmental conditions to maintain balance.


Humoral Epidemic Explanations

Epidemics were explained through the concept of miasmas or corrupted airs that disturbed the humoral balance collectively in populations. Changes in weather, pollution, and other environmental factors were believed to cause widespread humoral disturbances, leading to disease outbreaks.


Humoral Medicine across Regions

Humoral theory was adapted and elaborated in various cultural contexts. Greek physicians such as Hippocrates and Galen were foundational, while Islamic scholars like Avicenna expanded humoral concepts with detailed medical texts. In medieval Europe, humoral ideas remained central until the Renaissance and the rise of new medical paradigms.


Decline of Humoral Dominance

From the 17th century onward, humoral medicine gradually declined as anatomical studies, microscopy, and experimental methods revealed new understandings of disease causation. The discovery of pathogens and the germ theory of disease displaced the humoral framework.


Humoral Medicine after Germ Theory

Even after germ theory became established, humoral concepts persisted in some medical thought and practice, influencing ideas about body constitution and holistic health. However, modern medicine no longer relies on humoral theory as a scientific basis, viewing it instead as an important historical stage in the evolution of medical knowledge.
