Zona Fasciculata
- The middle and thickest layer, composed of long cords of cells arranged in straight columns.
- Cells contain abundant lipid droplets, mitochondria with tubular cristae, and smooth endoplasmic reticulum.
- Responsible for producing glucocorticoids, predominantly cortisol, which regulates metabolism, immune response, and stress adaptation.
- Cortisol secretion is under the control of ACTH from the anterior pituitary gland.
Zona Reticularis
- The innermost cortical layer, adjacent to the medulla.
- Cells are smaller, arranged in a network or reticular pattern.
- Produces adrenal androgens such as dehydroepiandrosterone (DHEA) and androstenedione, which serve as precursors for peripheral sex steroid synthesis.
- Also contributes to minor glucocorticoid production.
- Androgen secretion is stimulated by ACTH.
Adrenal Medulla
The medulla is composed primarily of chromaffin cells—modified postganglionic sympathetic neurons that synthesize and secrete catecholamines:
- Epinephrine (adrenaline), about 80% of catecholamine output.
- Norepinephrine (noradrenaline), about 20%.
These catecholamines are critical for the "fight or flight" response, increasing heart rate, blood pressure, blood glucose levels, and redirecting blood flow to vital organs and muscles.
Medullary Organization and Innervation
- Chromaffin cells are arranged in clusters and cords within a rich vascular network.
- The medulla receives direct preganglionic sympathetic input via splanchnic nerves.

- Superior suprarenal arteries (from the inferior phrenic artery),
- Middle suprarenal arteries (from the abdominal aorta),

- Venous drainage is via a single large suprarenal vein on each side, draining into the inferior vena cava on the right and the left renal vein on the left.
